Modified Blalock-taussig Shunt ----in- Hospital Shunt Occlusion: Experience at Nicvd

چکیده
A modified Blalock-Taussig shunt (mBTS) is a surgically placed graft between the systemic and pulmonary arterial circulations that redirects systemic blood flow into the pulmonary circulation. It is often placed in those with rightsided obstructive cardiac defects to increase pulmonary blood flow. Postoperative shunt patency is critical to adequate systemic oxygenation. Nevertheless, in the immediate hours after placement of a mBTS or before hospital discharge, a child may experience an episode of profound hypoxia and even cardiac arrest as a result of clotting of the shunt and consequent loss of pulmonary blood flow. Even with the use of aggressive postoperative anticoagulation therapy, thrombotic occlusion of a mBTS occurs in 3% to 9%. Experience with the modified Blalock-Taussig operation using polytetrafluoroethylene (Impra) grafts.
Between June 1978 and January 1982, 115 patients underwent 122 subclavian artery-pulmonary artery shunts using polytetrafluoroethylene (PTFE Impra) grafts. Forty-six of the patients had a ductus dependent pulmonary circulation, the patency of which was maintained by an infusion of prostaglandin E2 in 29 cases. There were nine hospital deaths, four of which were related to shunt failure. Five pa...

A four year old boy with pulmonary atresia and ventricular septal defect had an acute cyanotic episode three years after undergoing a right-sided, 6 mm diameter, modified Blalock-Taussig shunt. On admission no continuous murmur could be heard from the shunt and the typical high velocity, continuous flow profile of the shunt could not be identified by Doppler echocardiography. At catheterisation...

A case of transcatheter occlusion of a Blalock-Taussig shunt with a detachable silicone filled balloon is described. This 11 year old boy had previously had a repair of tetralogy of Fallot together with ligation of a large Blalock-Taussig shunt. Though there was a good surgical result, the shunt proved to be incompletely closed leaving a significant left to right shunt. As an alternative to a f...

A left-sided interposition graft (modified Blalock-Taussig anastomosis) was constructed with polytetrafluoroethylene in a three month old child with tetralogy of Fallot. This shunt thrombosed and a replacement shunt became the site of chronic Pseudomonas infection. The second anastomosis was excised and a third interposition graft was inserted on the right side because the anatomy was unsuitabl...

The modified Blalock-Taussig (BT) shunt is a common palliative procedure for congenital cyanotic heart disease (CHD) with diminished pulmonary blood flow(1). In developing countries, definitive surgery usually gets delayed due to limited resources and expertise(2). Children with cyanotic CHD and BT shunt often present to the pediatrician resulting in some of the complications of BT shunt being ...
